Pre-registration

New Patient Registration

Under the new GP contract of the 1st of April 2004, patients are registered at a surgery, rather than with an individual GP. Patients may choose their 'preferred' Doctor, however. You will be asked to attend a New Patient Medical with Kate Williams, our Health Care Assistant.

If you click in the Pre-registration tab above you can complete a registration form and medical questionnaire which can be sent electronically to us.

Practice Area

A map showing the Practice area is on view in the lobby. The Practices covers Whitchurch Town and the Bourne Valley and surrounding hamlets from Stoke through St. Mary Bourne to Longparish. The area extends West to the Andover ring road, and East to the edge of Overton
