New Patient Registration

Under the new GP contract of the 1st of April 2004, patients are registered at a surgery, rather than with an individual GP. Patients may choose their 'preferred' Doctor, however. You will be asked to attend a New Patient Medical with Kate Williams, our Health Care Assistant.

If you click in the Pre-registration tab above you can complete a registration form and medical questionnaire which can be sent electronically to us.

Patient Group

We are setting up an on-line patient group to encourage patients to give their views on the services we offer.  If you are interested in taking part, please go to the Patient Group tab on the right of the screen.

Practice Area

A map showing the Practice area is on view in the lobby. The Practices covers Whitchurch Town and the Bourne Valley and surrounding hamlets from Stoke through St. Mary Bourne to Longparish. The area extends West to the Andover ring road, and East to the edge of Overton

Non-English Speakers

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You must also complete and send the online medical questionnaire below.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
