Repeat Prescriptions

pill_bottleIf you are on long-term medication it has been agreed that you do not need to see your GP each time you need a Prescription. This page explains how to easily re-order your medicines.

Re-Order Forms

All prescriptions are now computerised and with each new prescription you will be given a re-order form. (Occasionally this will be 2 pages long). This form lists all your regular medication. Please keep this form safely - it will be helpful for any Doctor visiting you, if you are going to Hospital, or if you are travelling outside the Practice area. Spare forms are easily available from the Surgery. A typical re-order form is shown here. Please check the information on this form is correct. Please make any corrections when ordering your medicines or by contacting the surgery.

Review Date

There is a Review Date on your re-order form. This shows when your condition should be reviewed by your GP. In general terms you should visit your Doctor at least once a year if you are on regular medication.

To Re-order Your Medicines

Telephone requests for drugs are NOT accepted. However you may order your repeat medication on-line. Ask at Reception for a registration form and then follow the instructions. For your convenience you may collect your medicines from Lloyds Chemist in Whitchurch, or if you wish you may collect your prescription from the Surgery.

  1. Please indicate which method of collection you wish to use by ticking one of the boxes at the top of the form. Please do not tick the box DRUGS AT SURGERY as this service is only available to Rural Patients who live more than one mile away from a Chemist.
  2. Please tick the box alongside each drug you require. It will take 72hrs for your prescription to be processed so please allow adequate time.
  3. Place your re-order form in the box in the front lobby of the Surgery.

Pre-Payment Certificates

Prescriptions are generally issued in monthly amounts in accordance with National guidelines. If you are having more than fifteen prescriptions per year it may be worth purchasing a prepayment certificate. Details are available from the Post Office. Many people are exempt from prescription charges.

The chemist dispensing your medicines will help you with the regulations.

Rural patients on dispensing services and repeat prescriptions

Introduction

The Dispensary is open from 8.00am to 6.30pm for collection of medicines. You may wish to use the Dispensing service for both normal and repeat prescriptions. If you are on long-term medication it has been agreed that you do not need to see your GP each time you need a prescription

To order on-line ask at Reception for a registration form and then follow the instructions. If ordering by telephone, please do so between 11.00am and 1.00pm or 2.00 and 3.00pm. Please have your re-order form and computer number to hand. If ordering by post or leaving the
re-order form at the Surgery:

  • A: Please tick the appropriate box to collect drugs from the SURGERY or elsewhere.
  • B: Please tick the drugs you require.
